Mail the completed and signed form to: Vote By Mail Division, Registrar of Voters, PO Box 611750, San Jose, CA 95161 or FAX it to us at (408) 293-6002.

Your packet will be mailed to you 29 days prior to Election Day.
Once you receive your Vote By Mail packet, follow the easy instructions to vote your ballot. Seal the voted ballot in the return envelope provided to you. You must sign the envelope in order for your ballot to be counted. Mail your voted ballot back to us or turn it in on Election Day at any Santa Clara County polling place. Voters who do not vote their Permanent Vote By Mail Ballot may lose their Permanent Vote By Mail Voting status, but will continue to be registered to vote
