REFUND POLICY
If for any reason you cannot attend the workshop after sending a check, another person may be substituted, the fee applied toward other workshops offered by Learning Institute, or, your fee refunded, if notified by May 13, 2002.

CONTINUING EDUCATION UNITS FOR SAN JOSE STATE UNIVERSITY
For San Jose State University Continuing Education Units (CEU), complete CEU form and submit payment at the conference. Options from which to choose include (1) One CEU ($60) for Best Practices: Prevention Strategies That Work; (2) One CEU ($60) for Working with the Whole Teen: Toward a Developmental Model of Treatment and/or (3) .3 CEU ($20) for HANDS ON: Evaluation & Action Planning session.


CONTINUING EDUCATION (CE) CREDITS - 13 Hours for Teen Conference Only
To receive CE credit and a certificate, you must attend the entire Teen conference on both days, sign in at the beginning and end of each conference day and turn in completed evaluation sheets at the end of the conference. Official certificates will be mailed within four weeks. No partial credits are given for partial attendance. Non-refundable CE fee is $20; waived for DADS staff and contractors. If you wish CE credits, please fill in your certification or license number on the line following your licensure agency:

BBS - Course meets qualifications for 13 hours of continuing education credit for MFTs and LCSWs as required by the California Board of Behavioral Sciences (Provider Number PCE 797).

BRN - Provider approved by the California Board of Registered Nursing, Provider Number CEP12240 for 13 continuing education contact hours.

CAADAC - Provider approved by California Association of Alcoholism and Drug Abuse Counselors, Provider Number 4S-95-3050403 for 13 continuing education hours.
